Tissue Vigilance

If there is suspicion or evidence of a serious adverse reaction or an effect in the recipient possibly related to the safety and quality of the transplanted tissue, the physician should contact NBTS immediately by completing the “Tissue SAERs Health Care Facility” form and submitting it to NBTS on email [email protected].

Available to donors considered to need particular counselling.

When requested, our members of staff offer talks and informative sessions about blood donation and its process in schools, organizations, TV and radio programmes. Blood donors requesting access to their documents held by the National Blood Transfusion Department are to contact the Departments' FOI Officers on 2206 6215 or 2206 6204

Medical Questionnaire

To be discussed with the doctor prior to donation.

Under Age Consent Form

To be printed, signed by the guardian and presented by 17 year old donor. The donor must present a photocopy of the front and back of the guardian's I.D. Card. This photocopy must be endorsed by the guardian and will be retained together with the signed under age consent form by NBTS.

Directed Donations: Information and Consent

To be printed signed by the patient and pr​esented by the donor.

Autologous Transfusion Referral Letter

To be printed signed by the patient's consultant and presented at NBTS.
